Recalls

If you are an existing patient and received a recall reminder stating you are due for a follow up procedure please click on your treating doctor to submit your details and answers.

If you answered yes to the health questions, you will be required to see your doctor for a consultation prior to any procedures. If you answered no to the health questions and have a new referral, we will be in contact with you regarding what steps that need to be followed to go ahead with your procedure. You will also be required to notify us of what location you would like the procedure performed and what day is suitable.

*All patients are required to complete this form AND submit a new referral to our clinic before any procedure dates can be arranged. No bookings can be made without the TWO requirements.
